Job Title: Physical Therapist (PT) SNF / LTC

Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment in a skilled nursing facility (SNF) or long-term care (LTC) setting.

Job Summary:

The ideal candidate will have experience in providing physical therapy services to patients with various medical conditions, including those with physical disabilities or impairments. The Physical Therapist will be responsible for assessing patients, developing and implementing treatment plans, and educating patients and families on physical therapy methods.

Responsibilities:
  • Assess patients' medical conditions, functional capabilities, limitations, and restrictions.
  • Develop and administer treatment plans with specific goals determined according to patients' capacity and tolerance.
  • Provide direct patient care, including physical therapy interventions and exercises.
  • Collaborate with other healthcare professionals to plan and evaluate team goals for each patient.
  • Document and record patients' conditions and treatment plans.
Requirements:
  • Graduation from an accredited school of physical therapy.
  • Current state license as a Registered Physical Therapist or actively in the process of obtaining licensure.
  • Current CPR certification.
  • Ability to develop and implement treatment plans with specific goals.
