Job Summary:
We are seeking a skilled and compassionate Subacute Nurse to join our team at Complete Care at Bayshore, LLC. As a Subacute Nurse, you will play a vital role in providing exceptional nursing care to our residents, focusing on rehabilitation and recovery.
Responsibilities:
- Provide individualized care to residents, assessing their needs and developing personalized care plans to promote optimal health and well-being.
- Administer medications and treatments as prescribed, monitoring residents' conditions and adjusting care plans as needed.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care and support for our residents.
- Educate residents and their families about care plans, treatments, and health management, promoting informed decision-making and empowerment.
- Ensure adherence to all regulatory requirements and facility policies, maintaining a safe and compliant environment for our residents.
- Support the overall well-being of residents by fostering a positive and respectful environment, promoting dignity and autonomy.
Requirements:
- High School Diploma or equivalent.
- Previous LTC/SNF experience preferred.
- RN certification in NJ.
