A Patient Care Manager is needed at Avera McKennan Hospital to facilitate patient transitions from acute settings to appropriate levels of care. This role emphasizes career growth, teamwork, and a positive work environment within a Magnet Recognized organization.
Key Responsibilities:- Facilitate the plan for an orderly transition of patients from the acute area setting to the next appropriate level of care.
- Work closely with physicians and payors to assure proper use of fiscal resources.
- Serve as liaison with physicians and staff to assure ongoing continuity.
- Graduate from a CCNE or NLN accredited nursing program strongly preferred.
- Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
- Professional certification preferred.
- Competitive pay: $32.00 - $46.50 per hour
