Nurse Case Manager Position

The Registered Nurse Case Manager is a key member of our team at Nascentia Health, responsible for providing direct patient care and coordination of overall home care services. This role requires a strong nursing background, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities:
Ensure quality and safe delivery of home health services, including case managing client caseloads and developing plans of care based on client assessments and OASIS evaluations.
Complete all OASIS evaluations and documentation, ensuring that all records meet quality standards and are completed in a timely manner.
Coordinate with LPNs, providing guidance and education on client care and working in conjunction with the Clinical Supervisor and Patient Care Coordinator to efficiently manage patients using a team approach.
Supervise paraprofessional staff, including home health aides, in performing designated tasks related to patient care and communicating relevant information to the home health aide department.
Requirements:
Current New York State registered professional nurse license (degree must be from an accredited school of nursing). BSN a plus.
Valid driver's license and auto insurance, as well as a vehicle that can be used for work (we have a Fleet Program for field clinicians).
Self-directed and organized, with the ability to work independently in a fast-paced environment.
Previous home care/case management experience a plus.
Proficient computer skills, including knowledge of electronic medical records software, with the ability to learn new software.
Exceptional oral, written, and verbal communication skills.
Physical Requirements:
Ability to sit or stand at a desk 90% of the day.
Ability to perform tasks involving physical activity, including pushing/pulling, lifting/carrying, and patient transfers.
Ability to drive in all weather conditions.
Visual/hearing ability sufficient to comprehend written or verbal communication.
Ability to use a telephone and communicate over the phone.
