Key Responsibilities:
  • Assess, plan, coordinate, implement, and document care for assigned residents, prioritizing their unique needs and preferences.
  • Build strong relationships with clients, caregivers, and family members, fostering open communication and trust.
  • Develop and maintain team and client schedules, ensuring seamless care delivery.
  • Orient new team members to department operations and policies, promoting a culture of excellence.
  • Provide guidance and recommendations for additional training as needed, supporting the growth and development of the team.
  • Supervise Home Service Workers, ensuring they receive the necessary support and resources to deliver exceptional care.
  • Perform skilled-nursing visits for clients when necessary, addressing their medical needs and concerns.
  • Manage client and caregiving information in our information management systems, ensuring accuracy and reliability.
  • Communicate effectively with clients, caregivers, and family members, particularly when changes are observed in clients' condition, fostering strong working relationships and client outcomes.
  • Complete documentation and paperwork in a timely manner, adhering to department policies and procedures.
  • Attend in-services, meetings, and educational training, staying up-to-date on industry best practices and company policies.
Requirements:
  • Valid driver's license, reliable access to a car, and active car insurance.
  • Successful passing of a drug test and IDPH fingerprint criminal background test.
  • Valid Illinois Registered Nurse License.
  • Acute Care Nursing – 1 year minimum preferred.
  • Home Health/Care – 1 year minimum preferred.
  • BLS required.
