Job Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) and/or Registered Nurse (RN) to join our team at the Visiting Nurse Association of Florida (VNA) in St. Lucie West, Florida.

Job Summary

The successful candidate will work with referral sources from the healthcare community to implement services that meet the needs of individuals and families in their homes. This includes completing physical assessments, determining patient home care needs, and developing and implementing a plan of care for each patient using VNA of Florida's case management model.

Responsibilities

  • Complete initial assessments of patients to determine home care needs
  • Develop and implement plans of care for each patient using VNA of Florida's case management model
  • Work with referral sources from the healthcare community to implement services that meet the needs of individuals and families in their homes
  • Provide one-on-one care to patients in their homes
  • Work independently in the field
