Job Summary:
The Physical Therapist will independently evaluate, determine treatment strategies, deliver and/or delegate services to identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, with emphasis on physical dysfunction.
Key Responsibilities:
- Conduct thorough examinations and evaluations of patients' levels of function, including strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait, and functional abilities in activities of daily living.
- Develop individualized patient-centered treatment plans to promote optimum movement function and safety.
- Implement, deliver, and delegate patient care by utilizing appropriate interventions and clinical decision making, including modalities such as heat, cold, light, water, sound, electricity, mobilization, and therapeutic exercise.
- Collaborate with other care providers to coordinate patient care, including Rehab Therapists, Nurses, Social Workers, and participate in patient transition planning and/or patient discharge planning.
Requirements:
- Graduate of a Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree.
- Active or temporary license in the state of Connecticut.
- CPR certification.
