We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Northern Arizona Healthcare Corporation. As a Patient Care Coordinator, you will play a vital role in providing exceptional patient care and support to our patients and their families.
Key Responsibilities- Patient Care
- Assist patients with daily living activities, such as toileting and mobility, under the direction of supervising clinicians.
- Monitor patient skin integrity and report any concerns to supervising clinicians.
- Assist with patient transfers, including transfers to and from wheelchairs, as needed.
- Assist patients with donning and doffing compression garments, as needed.
- Discharge Support
- Assist patients with discharge logistics, including securing DME and coordinating with community agencies.
- Support and reinforce discharge instructions to patients and their families.
- Administrative Support
- Answer and evaluate incoming administrative line telephone calls, prioritizing and communicating effectively with callers.
- Assist with clerical duties, including maintaining unit documents and charts.
- Document patient data and care provided, reporting abnormal findings to supervising clinicians.
- High School Diploma or GED
- Graduate of a Nursing Assistant Program
- Current, unrestricted CNA or LNA license in the State of Arizona
- Basic Life Support (BLS) certification
- Fingerprint Clearance Card application number (if not already obtained)
- Demonstrated ability to collaborate with interdisciplinary teams and community agencies
- Knowledge of available community resources and post-discharge resource planning
- Competence in using computers and basic software programs
This position requires working in a fast-paced healthcare environment, with frequent interactions with patients, families, and healthcare professionals. The ideal candidate will be able to work effectively in a team environment, prioritize tasks, and maintain a high level of professionalism and compassion.
