About the Role:

We are seeking a compassionate and dedicated Home Health Care Professional to join our team at Applied Palliative and Hospice Services, Inc. As a member of our home care team, you will work under the supervision of a registered nurse or therapist, providing personal care services to patients in their homes.

Key Responsibilities:

  • Provide hands-on personal care, including bathing, back rubs, oral hygiene, shampoos, changing bed linen, assisting with dressing and undressing, skin care to prevent breakdown, and toileting activities.
  • Plan and prepare nutritious meals for patients.
  • Assist with feeding patients as needed.
  • Take and record vital signs, such as temperature, pulse, respiration, and blood pressure, when ordered.
  • Assist patients with ambulation and exercise according to their plan of care.
  • Perform range of motion and other simple procedures as an extensional therapy service, if ordered.
  • Participate in patient safety needs and use equipment safely and properly.
  • Report patient condition and significant changes to assigned nurses.

Requirements:

  • Dedicated and compassionate individual with a passion for providing quality care to patients in their homes.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
