About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Evaluate and provide speech-language pathology services to patients in their homes, utilizing your expertise to assess and treat disorders of speech, voice, language, hearing, and swallowing.
  • Develop and implement individualized treatment plans, working closely with patients, families, and healthcare staff to ensure comprehensive care.
  • Administer and interpret diagnostic tests, and provide instruction and training to patients in the use of alternative communication systems when necessary.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
  • Maintain accurate and detailed records of patient progress, providing timely and informative reports to supervisors and healthcare professionals.
Requirements
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA.
  • Minimum of six months experience as a speech therapist/speech-language pathologist, with a preference for those with home health experience.
  • Current and unrestricted license to practice as a speech-language pathologist in the state of employment.
  • Current CPR certification.
  • Excellent organizational and communication skills, with the ability to work effectively in a fast-paced environment.
