Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will provide direct patient care services, act as a liaison between physicians and patients/families, and empower caregivers through education and support.
Responsibilities- Provide direct patient care services to hospice patients and supportive care to patients' families
- Act as primary liaison between physician, patient/family, and hospice team
- Empower patient's caregivers through education and support
- Manage and collaborate with interdisciplinary team to provide comfort, peace, and dignity
- Evaluate patient and family needs and assist in preparation and execution of plan of care
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Current driver's license and automobile insurance
- Current CPR certification
