Key Responsibilities- Develop and implement comprehensive care plans for patients, incorporating medical interventions and measurable goals and outcomes.
- Educate patients and their family members on specific care needs, promoting independence and self-management.
- Train primary caregivers to ensure optimal functioning levels for each patient, providing guidance and support as needed.
- Coordinate the continuum of care through timely documentation and communication with physicians and other caregivers.
- Provide skilled nursing care in patients' homes, utilizing a range of skills including phone triage, patient education, observation/assessment, wound care, infusions, and more.
- An active RN license in the state of application.
- Valid CPR certification.
- Preferred: Medicare Skilled Nursing experience, basic understanding of Oasis, and 1-year RN experience in a healthcare setting.
