Job Title: After Hours LPN

Job Summary:

We are seeking a skilled and compassionate LPN to join our team as an After Hours Nurse. This is a full-time position working three shifts per week, every 4th weekend, and includes a holiday rotation.

Responsibilities:

  • Provide competent nursing care to assigned patients through skilled symptom management, emotional support, and timely response to patient and family needs.
  • Conduct routine home care visits, end-of-life care, and referrals as needed.
  • Coordinate admission of home-care patients to in-patient settings, as required.
  • Work evenings and weekends as needed to meet patient and family needs.
  • Complete all documentation as required by the organization, in compliance with agency policy, accreditation bodies, and governing requirements.

Requirements:

  • Graduation from an approved School of Nursing.
  • Current LPN licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.
  • Valid driver's license.
  • Excellent customer service skills.
  • Detail-oriented with excellent communication, documentation, and organizational skills.

Working Conditions:

The After Hours Nurse will work independently without supervision, with the ability to lift 25 lbs of supplies or equipment. Travel to other sites may be required, which may involve inclement weather.
