Job Summary

UPMC Life Changing Medicine is seeking a skilled Mental Health Counselor I to join our team. As a key member of our Residential Programs, you will provide counseling and casework services to individuals, promoting their overall well-being and quality of life.

Key Responsibilities
  • Provide counseling services to individuals, including crisis intervention and de-escalation techniques.
  • Develop and implement individualized treatment plans, working closely with consumers to achieve their goals.
  • Assess and document consumer progress, updating treatment plans as needed.
  • Collaborate with treatment team members to ensure comprehensive care and support.
  • Provide education and guidance on daily living skills, financial management, and socialization.
  • Assist consumers with scheduling and attending medical and dental appointments, ensuring proper documentation and follow-up.
  • Develop and maintain relationships with community resources, including pharmacies, grocery stores, and healthcare providers.
  • Participate in regular treatment team meetings and contribute to the development of treatment plans.
  • Maintain accurate and thorough documentation of consumer progress and treatment plans.
Requirements
  • High School diploma or equivalent required; related experience preferred.
  • Valid PA driver's license and authorization to initiate a Driver History Review.
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
  • Comprehensive Crisis Management (CCMC) certification.
  • Act 34 clearance.
  • OAPSA clearance.
