Join Our Team as a Home Health RN

We are seeking a skilled and compassionate Registered Nurse to join our team at Cambridge Home Health Care. As a Home Health RN, you will have the opportunity to provide high-quality care to patients in their homes, making a meaningful difference in their lives.

Key Responsibilities:

  • Provide clinical services to patients in their homes, following the plan of care and in accordance with federal and state regulations.
  • Conduct thorough clinical assessments to ensure patient eligibility and medical necessity for services.
  • Develop and revise individualized plans of care, collaborating with other community providers as needed.
  • Ensure plan of care frequency and duration meets patient needs, initiating revisions as necessary with physician approval.

Requirements:

  • Current RN licensure in the state of practice.
  • One year of clinical experience as an RN, with recent experience as an LPN considered in some states.
  • Current CPR certification.
