Job Summary

The OB Nurse Case Manager plays a vital role in coordinating patient care and ensuring seamless transitions between healthcare services. This position requires a skilled nurse with strong communication and organizational abilities to facilitate effective care coordination and advocacy for patients and families.

Key Responsibilities
  • Collaborate with healthcare teams to assess patient needs and develop comprehensive care plans
  • Coordinate patient transfers and transitions between healthcare services
  • Provide education and support to patients and families to promote self-management and empowerment
  • Advocate for patients and families to ensure access to necessary resources and services
  • Maintain accurate and up-to-date patient records and documentation
Requirements
  • Graduate of an accredited school of nursing
  • Current RN licensure in any state, with Alaska licensure required within six months of hire
  • Current BLS certification
  • PALS, NRP, TNCC, ENPC certifications preferred
  • Case Management Certification preferred
  • Two years of recent nursing experience, with case management experience preferred
