Advanced Practice Provider

Nationally certified Advanced Practice Providers are sought by Euclid Hospital to join their Medicine team. These professionals will be responsible for providing primary and specialty care in various healthcare settings, working collaboratively with the healthcare team.
Responsibilities:Conduct thorough medical histories, perform complete physical examinations where indicated, initiate appropriate lab and radiology tests, and scrutinize lab data to establish a patient's current health status and develop a treatment plan.
Perform and interpret common laboratory, radiologic, cardiographic, and other routine diagnostic procedures as credentialed and privileged by the Medical Executive Committee and the Governing Body of applicable hospitals.

Prescribe and monitor medications as allowed by license through applicable state boards and Cleveland Clinic policies.
Educate patients on their disease, treatments, related drug and treatment side effects, and hazards.
Participate in medical team rounds, collaborate with nursing, medical, and other healthcare team members regularly to ensure quality patient care.
Refer patients to specialists as necessary for consultation or specialized health resources and treatment.
Ensure continuity of care by serving as a liaison between patients and other members of the multi-disciplinary care team or with other specialty areas as necessary.
Participate in quality monitoring by reviewing records and treatment plans for patient outcomes on a periodic basis to assure quality care.
Facilitate appropriate length of stay, discharge planning, and compliance with regulatory standards for inpatient management.
Support the professional development of the healthcare team.
Promote the translation of evidence-based practices, guidelines, and research.
Maintain accurate and up-to-date knowledge of medical information and technology.
Must possess a Master's Degree in Nursing (MSN) from an accredited program, with some exceptions based on work setting.
National certification as a Nurse Practitioner is required, obtained from a nationally accredited organization recognized by the state Board of Nursing.
Cleveland Clinic requires licensure as a Registered Nurse (RN) in applicable states, as well as Basic Life Support (BLS) certification through the American Heart Association.
Additional certifications may be required based on specific work settings, such as Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS).
Must have arrangements with at least one physician in accordance with requirements set forth by the State Board of Nursing in applicable states.
Credentialed and privileged by the Medical Executive Committee and the Governing Body of applicable hospitals.
