At Diversicare of Marion, we are seeking a highly skilled and compassionate Director of Memory Care Services to join our team. As a key member of our leadership team, you will be responsible for overseeing the Memory Care program and ensuring that our residents receive the highest level of care and support.
Responsibilities:- Develop and implement a comprehensive Memory Care program that meets the unique needs of our residents
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication
- Provide education and training to staff on dementia care and management
- Conduct regular assessments and evaluations to ensure the effectiveness of our Memory Care program
- Develop and maintain relationships with community partners and healthcare providers to ensure seamless transitions of care
- Preferred licensure in nursing or social services discipline (LVN, LPN, RN or BSW)
- Excellent communication and interpersonal skills
- Experience working with residents with dementia diagnosis and understanding of disease process, stages of dementia and programming needs of dementia population
- Ability to direct and perform a variety of activities and to respond to varied needs of the residents
