Caregiver Assistant

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents and their families.
Responsibilities- Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
- Develop and implement Individualized Service Plans (ISPs) in collaboration with the community team.
- Ensure the highest level of care and services are provided to residents, responding to their needs and preferences.
- Participate in the development of ISP and monthly updates, ensuring the resident's care plan is aligned with their needs and goals.
- Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude, flexibility, and ability to work in a team environment.
- High School diploma/GED accepted and may be required per state regulations.
- Previous experience working with seniors preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
