CommuniCare Health Services Corporate is seeking a skilled professional to join our team as a Patient Assessment Coordinator.

Job Summary:

The Patient Assessment Coordinator will play a critical role in identifying patients for potential admission into our skilled nursing facilities and maintaining positive relationships with current referral sources.

Key Responsibilities:
  • Perform comprehensive clinical assessments to determine patient eligibility for admission
  • Coordinate the transmission of clinical and benefit information from referral sources to our admissions teams
  • Provide exceptional customer service and communicate effectively with patients, families, and healthcare professionals
Requirements:
  • Current, valid, and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, or Social Work Degree, or other medical credential
  • Three years of treatment experience in a Long Term Care setting preferred
  • Previous clinical screening experience preferred
  • Recent healthcare experience in Long Term Care preferred
  • Knowledge of local medical community preferred
