Job Summary:The Patient Assessment Coordinator will play a critical role in identifying patients for potential admission into our skilled nursing facilities and maintaining positive relationships with current referral sources.
Key Responsibilities:- Perform comprehensive clinical assessments to determine patient eligibility for admission
- Coordinate the transmission of clinical and benefit information from referral sources to our admissions teams
- Provide exceptional customer service and communicate effectively with patients, families, and healthcare professionals
- Current, valid, and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, or Social Work Degree, or other medical credential
- Three years of treatment experience in a Long Term Care setting preferred
- Previous clinical screening experience preferred
- Recent healthcare experience in Long Term Care preferred
- Knowledge of local medical community preferred
