We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at the Veterans Affairs, Veterans Health Administration. As an LPN, you will play a vital role in providing direct support to the Patient-Aligned Care Team (PACT) in the Primary Care Outpatient Clinic.
Responsibilities- Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
- Collaborate with the PACT team to develop comprehensive health care plans and care management plans for Veterans assigned to the panel.
- Manage clinic flow, including vital signs, treatments, and patient communication.
- Assist with pre-visit phone calls to Veterans to complete reminders and obtain reason for appointment.
- Review daily clinic schedule for patient care needs and ensure smooth clinic flow.
- Act as patient advocate, ensuring patient understanding of visit content and follow-up instructions.
- Participate in telehealth duties, including managing required screenings, completing clinical reminders, and documenting patient information.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
- Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States, or District of Columbia.
- English language proficiency, with the ability to communicate effectively with patients, families, and healthcare team members.
The work environment is fast-paced and dynamic, with a focus on providing high-quality patient care. The LPN will work closely with the PACT team to ensure seamless care transitions and effective communication with patients and families.
What We Offer- Competitive salary and regular salary increases.
- 37-50 days of annual paid time off per year, including 13 days of sick leave and 11 paid Federal holidays.
- Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.
- After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
- Child care subsidy available after 60 days of employment, with a total family income below $144,000.
- Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.
