About the Role:

  • You will perform routine and emergency assessments, providing physical and emotional care to support patients and their families during times of crisis.
  • You will educate patients and caregivers on disease processes, symptom control, wound care, and other relevant topics to promote informed decision-making and optimal outcomes.
  • You will work closely with our IDT members to identify patient needs and develop effective care plans, ensuring seamless coordination and communication throughout the care continuum.

Requirements:

  • Master's degree in Nursing required; minimum 1 year of nursing practice or equivalent experience preferred.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Active NPI number and DEA certification (if applicable) required.

Estimated Salary: $120,000 - $150,000 per year, depending on location and experience. We offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
