About the PositionWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time provider. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, using our proprietary assessment tool to identify potential health risks and develop personalized care plans.
Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Provide health education and counseling to patients and their families
- Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
- Document patient information and outcomes in our electronic health record system
- Master's degree in nursing or related field
- Current RN and NP licensure in state of practice
- Board certification in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record systems
