Job Summary:

The Physical Therapist will evaluate and treat patients with the use of medically prescribed physical therapy programs. This includes supervising Physical Therapy Assistants, making initial admission/evaluation visits, and notifying assigned Clinical Team Manager of OASIS scores within established timelines. The ideal candidate documents observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.

Responsibilities:

• Evaluate and treat patients with medically prescribed physical therapy programs
