Job Title: LPN Mobile Crisis Services

Job Summary:

Elwyn is seeking a skilled Licensed Practical Nurse (LPN) to join our Mobile Crisis Services team. As a member of our interdisciplinary team, you will provide critical care and support to individuals in crisis, working collaboratively with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:

  • Conduct thorough nursing evaluations at the site of the encounter, gathering essential information about the client's medical history, current concerns, medications, and recent hospitalizations.
  • Collaborate with the Registered Nurse (RN) to develop and implement a treatment plan, ensuring continuity of care and effective communication with the client and their support network.
  • Administer medications, document all details of the encounter, and maintain accurate records in the client's chart and electronic health record.
  • Facilitate transfer to the hospital or Emergency Department as needed, communicating with emergency services and updating the RN on the situation.
  • Present pertinent data obtained during the client encounter to the Interdisciplinary Team (IDT), ensuring seamless communication and continuity of care.
  • Report any observed medication and treatment side effects or abnormal laboratory values to the RN immediately for further observation and follow-up care.
  • Document all behavioral and physical changes in the client's status and other pertinent information in the nursing progress notes and electronic health records.

Requirements:

  • Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
  • One (1) year of psychiatric, medical surgical, or chemical dependency nursing experience.
  • Knowledge and understanding of program regulating bodies (e.g., Office of Mental Health & Substance Abuse Services OMHSAS, Department of Behavioral Health) preferred.
  • Ability to work in a fast-paced environment, manage and prioritize multiple priorities, and demonstrate strong attention to detail.
  • Excellent customer interaction, collaboration, presentation, and written and verbal communication skills.
  • Current CPR certification and ability to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols.
  • Basic experience with Microsoft Office applications, including Word, Excel, and Outlook.
  • Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred.
  • Valid driver's license in the state of residence, three (3) years driving experience in the United States, and acceptable driving record.
