Job Description

Job Summary: We are seeking a highly skilled and compassionate Nurse Home Visitor to join our team at the City of New York. As a key member of our public health team, you will play a critical role in improving maternal and child health outcomes in our city.

Key Responsibilities:

  • Provide home visiting services to families from pregnancy through the postpartum period, focusing on maternal and child health, mental health, and chronic disease prevention.
  • Conduct comprehensive assessments of family needs and develop individualized plans to address those needs.
  • Collaborate with community stakeholders, healthcare providers, and other agencies to ensure seamless service delivery and coordination.
  • Provide education and support to families on healthy behaviors, parenting skills, and community resources.
  • Participate in program evaluation and quality improvement activities to ensure the effectiveness of our services.

Requirements:

  • Bachelor's degree in Nursing from a regionally-accredited college or university.
  • Current registration to practice as a Registered Professional Nurse in New York State.
  • Excellent interpersonal, communication, and presentation skills.
  • Experience in public health, particularly in maternal and child health, breastfeeding, and/or early childhood development.
  • Fluency in English and Spanish or French.

Preferred Qualifications:

  • Master's degree in Nursing or a related field.
  • Experience in home visiting or a related field.
  • Knowledge of community resources and services.
