Care Manager Job Description

CDS Life Transitions is seeking a skilled Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring the health and well-being of our members by assessing their physical, social, and environmental needs.

Key Responsibilities:

  • Oversee a caseload of managed care members
  • Review and evaluate member assessments and UAS information
  • Develop comprehensive service plans
  • Make referrals to appropriate health and community resources
  • Review and authorize requests for services
  • Maintain clear and effective communication with members
  • Educate members and caregivers on covered services and benefits
  • Evaluate member responses to interventions and make modifications as necessary
  • Collaborate with interdisciplinary teams
  • Document all interventions
  • Maintain member privacy and confidentiality
  • Adhere to ethical, legal, and regulatory standards

Requirements:

  • New York State Licensed Registered Nurse or Licensed Master's Social Worker
  • Minimum five years in a healthcare-related field
  • Ability to work independently and motivate others
  • Ability to communicate effectively, both orally and in writing
  • Reliable transportation and a valid NYS driver's license
