We are seeking a skilled and dedicated LPN to provide specialized nursing care to technology-dependent patients in their homes.

In this role, you will work closely with the healthcare team to ensure comprehensive patient care, perform assessments, administer medications, and educate patients and families.

Key Responsibilities:

  • Develop long-term relationships with patients and their families
  • Provide high-tech nursing care to patients with complex medical conditions
  • Collaborate with the healthcare team to ensure comprehensive patient care
  • Perform assessments, administer medications, and educate patients and families

Requirements:

  • Current LPN license in Vermont
  • Two or more years of directly related home/public health, hospice, and/or maternal/child health nursing experience
  • Previous home care experience a plus
  • Possesses strong clinical skills and a commitment to patient-centered care
