About the Role

We are seeking a highly skilled Medical Social Worker to join our team at Satellite Healthcare, Inc. As a Medical Social Worker, you will play a vital role in supporting the social and emotional well-being of our patients living with chronic kidney disease and undergoing dialysis treatments.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients' needs and develop personalized care plans.
  • Collaborate with the multidisciplinary team to provide holistic care and support to patients and their families.
  • Develop and implement strategies to address patients' social and emotional challenges, promoting their overall well-being and quality of life.
  • Provide education and support to patients and their families on chronic kidney disease management, treatment options, and self-care techniques.
  • Participate in care planning and team review to ensure patients receive comprehensive and coordinated care.
Requirements
  • Master's Degree in Social Work from an accredited CSWE institution.
  • MSW required; LCSW preferred; ASW (CA) recommended; Licensure/LMSW required in states with licensure (TX, TN).
  • One (1) year of work experience in a behavioral or healthcare-related setting, preferred.
  • Preferable experience in other health and human services fields; Medical experience is highly desirable.
