Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will be responsible for delivering high-quality patient care in a variety of settings. Key Responsibilities
  • Deliver post-acute care to patients in various settings
  • Monitor patients' physical and mental well-being, tracking vital signs and documenting health concerns
  • Perform intakes of patients, documenting their medical histories and health concerns
  • Review patients' medical records and charts
  • Administer and verify medications, maintaining accurate records
  • Contribute to a safe and comfortable environment for patients
  • Assist with IV set up, vaccinations, and phlebotomies
  • Adhere to care plans for patients, ensuring their needs are met
  • Support medical staff with orientation and training
Requirements
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience as an LPN or in a similar role
  • Ability to lift a minimum of 50 lbs
