The Clinical Care Leader (CCL) plays a vital role in delivering personalized nursing care within the home environment by employing the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. This position requires collaboration with a multidisciplinary team to effectively plan, execute, and assess care. Responsibilities include documenting resident histories, symptoms, medications, and care provided in accordance with established standards and regulations. The CCL utilizes technology to enhance the quality of resident care, collaborates with team members, effectively delegates tasks, supervises unlicensed personnel, and maintains overall nursing accountability for assigned residents while taking on leadership roles in departmental and organizational committees.
Effective communication skills are essential for collaborating, negotiating, and resolving conflicts with peers, team members, residents, and their families. Strong clinical reasoning and critical thinking abilities are necessary to analyze data and respond appropriately to evolving situations, including care coordination, resident assessments, education, triage, and various nursing interventions.
The CCL operates within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules relevant to their state of practice and licensure. The Sanford Professional Nursing Practice adheres to the Scope and Standards of Practice and the Code of Ethics for Nurses as published by the American Nurses Association, which serves as the foundation for nursing care delivery and professional conduct.
Qualifications
A Bachelor's degree in nursing is preferred, along with graduation from a nationally accredited nursing program. A minimum of two years of experience in long-term care or geriatric nursing is preferred, and experience with the Minimum Data Set (MDS) is advantageous but not mandatory.
The candidate must hold an unencumbered RN license with the State Board of Nursing in their practice area and/or possess multistate licensure if applicable. Required competencies and certifications must be obtained and maintained. Depending on the location, a valid driver's license may be necessary. Basic Life Support (BLS) certification is required.
Experience with the Resident Assessment Instrument (RAI) process is preferred but not required.
