We are seeking a skilled LPN to provide high-quality care to patients in their homes. As a PRN Continuous Care LPN, you will work under the supervision of a RN to provide supportive care and emotional support to patients and their families.

Key Responsibilities
  • Provide continuous bedside clinical observations and prescribed care to patients as directed in the plan of care.
  • Process physician orders to maximize patient comfort and report significant changes in patient condition and family needs as needed.
  • Collaborate with the RN case manager to develop and implement patient care plans.
Requirements
  • Graduate of an approved nursing program and current Florida LPN license.
  • Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
