About This Role

We're seeking a dedicated and compassionate Care Manager Associate to join our team at Sunrise Senior Living. As a Care Manager Associate, you'll have the opportunity to make a meaningful impact in the lives of our residents and their families.

Job Summary:

The Care Manager Associate is responsible for providing exceptional care and support to our residents, including assisting with activities of daily living, monitoring resident conditions, and developing personalized care plans.

Key Responsibilities:

  1. Resident Support: Assist residents with daily tasks, such as bathing, dressing, and grooming, while promoting independence and dignity.
  2. Communication: Maintain open and transparent communication with residents, family members, and healthcare professionals to ensure seamless care coordination.
  3. Documentation: Accurately document resident information, including medical history, medications, and progress notes, in accordance with Sunrise's policies and procedures.
  4. Collaboration: Work collaboratively with the care team to develop and implement individualized care plans that meet each resident's unique needs and preferences.

Requirements:

  • High school diploma or equivalent required.
  • Previous experience working in healthcare or a related field preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.

  • A competitive salary range of $45,000 - $65,000 per year, depending on experience and qualifications.
