Job Description

We are seeking a skilled LPN to join our team as a Home Health Nurse for a travel assignment in Georgia. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress and communicate with patients, families, and healthcare teams to ensure continuity of care.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current Georgia Nursing License
  • Minimum of 1 year recent home health nursing experience
  • Electronic Charting experience
  • Medicare Certified Agency Experience
