About the Role

We're seeking a highly skilled and compassionate Nurse Case Manager to join our team As a key member of our clinical staff, you'll play a critical role in coordinating patient care, ensuring seamless transitions between settings, and promoting optimal health outcomes. Your dedication, empathy, and attention to detail will make all the difference in the lives of our patients and their loved ones.

Your primary responsibility will be to work closely with interdisciplinary teams to develop and implement individualized care plans, addressing the complex needs of seniors with chronic conditions. You'll serve as a patient advocate, bridging the gap between healthcare providers, social services, and community resources to ensure comprehensive support.

Key Responsibilities
  • Patient Assessment and Planning: Conduct thorough assessments, identify patients' needs, and develop tailored care plans to address physical, emotional, and social requirements.
  • Care Coordination: Collaborate with healthcare providers, family members, and caregivers to facilitate smooth transitions between care settings, ensuring continuity and minimizing risks.
  • Advocacy and Education: Serve as a patient advocate, providing education and guidance on self-management, medication adherence, and healthy lifestyle choices.
  • Community Resource Navigation: Connect patients and families with local resources, services, and support networks to enhance their quality of life.

Educational Requirements: Associate degree in Nursing required; Bachelor's degree in nursing or related field preferred. Current RN licensure in the state of employment is essential.

Experience: Minimum 2 years' clinical experience; at least 1 year in utilization review, case management, home health, or discharge planning. Certified Case Manager (CCM) certification is a plus
