Position Overview

We are looking for a dedicated Community Health Support Specialist with expertise in substance use disorders.

Location: Pickaway County, OH

Qualifications:

  • Valid license for substance use disorder psychotherapy issued by the relevant state board.
  • Bachelor's degree or a minimum of three years of relevant experience.
  • Strong cultural competence and awareness.
  • Experience working with individuals and families with multiple needs.
  • Comprehensive understanding of community service frameworks.
  • Willingness to engage in and facilitate collaborative team-building activities.
  • Effective communication skills, both verbal and written.
  • Proficient in operating within an automated, internet-based office environment.

Key Responsibilities:

  • Evaluate the strengths and requirements of individuals seeking assistance for substance use issues.
  • Conduct assessments to determine appropriate levels of care for treatment.
  • Coordinate and connect individuals with necessary resources and services.
  • Facilitate both group and individual psychotherapy sessions focused on substance use disorders.
  • Engage in proactive outreach to referrals and individuals experiencing treatment lapses.
  • Maintain accurate and timely documentation of client interactions and related activities.
  • Participate in team meetings, collaborate with community partners, and engage in programs that support the individuals served.
  • Foster collaborative relationships with behavioral health teams and other departments within the organization.
  • Meet established productivity requirements for billing as set by the organization.
  • Develop partnerships and networks to enhance substance use programs and strengthen community ties.
  • Perform additional duties as assigned.
