Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will provide personalized care to patients in the comfort of their own homes, helping them maintain their independence and quality of life.

Key Responsibilities
  • Provide direct patient care under the direction of a registered nurse and according to the patient's care plan.
  • Assist patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake.
  • Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
  • Help patients maintain good personal hygiene, including bathing, grooming, skin care, and other activities.
  • Assist with feeding patients, including communicating basic nutrition principles and observing food and fluid intake.
  • Provide mobility assistance, including transfers, ambulation, and positioning/turning, as per the patient's care plan.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
Requirements
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Meets applicable state certification requirements.
  • Valid driver's license, auto insurance, and reliable transportation.
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
