Job Summary

The Ambulatory Licensed Practical Nurse is a valued member of a dynamic interdisciplinary team at AtlantiCare. They utilize their care process in line with their license and within their scope of practice. As an Ambulatory Licensed Practical Nurse, their responsibilities include case finding, health teaching, counseling, and providing care under the guidance of a Registered Nurse or Physician. They contribute to the coordination of the interdisciplinary plan of care and ensure alignment with the medical treatment plan.

Key Responsibilities

  • Conduct thorough assessments of patients to identify health needs and develop individualized care plans.
  • Provide direct patient care, including administering medications, performing treatments, and educating patients on self-care techniques.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient information accurately and thoroughly, adhering to organizational guidelines and standards.
  • Participate in performance improvement initiatives and prioritize professional growth to enhance patient outcomes.

Requirements

Education: Completion of an accredited nursing program.

License/Certification: Current LPN licensure in New Jersey and BLSHCP certification.

Specialty Experience: Family Medicine/Urgent Care, Internal Medicine, Pediatric Pulmonary, Trauma Outpatient, Clinic Services, Infectious Disease, Mission Services.

Work Environment: Potential exposure to hospital hazards, including infectious diseases and hazardous substances. Involves physical activities like standing, walking, lifting, and pushing heavy equipment. Some roles require color distinction ability.

Reporting Relationship: Reports to patient care unit leadership.
