Job Title: Licensed Practical Nurse

Join the Veterans Affairs, Veterans Health Administration team as a Licensed Practical Nurse (LPN) in our Community Living Center (CLC). As an LPN, you will provide direct care to our residents, working closely with the Registered Nurse and other healthcare professionals to ensure the highest quality of care.

Responsibilities:
  • Provide direct care to residents, including medication administration, wound care, and vital sign monitoring.
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct regular assessments of residents' physical and emotional needs, reporting any changes or concerns to the Registered Nurse.
  • Administer medications and treatments as prescribed by the physician, accurately documenting any reactions or side effects.
  • Participate in the development and implementation of resident-centered goals and outcomes, focusing on rehabilitation, restorative, and supportive care.
Requirements:
  • Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • Graduation from an approved school of practical or vocational nursing, or equivalent experience.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with residents, families, and healthcare professionals.
Work Environment:

The LPN will work in a dynamic and supportive environment, providing care to residents in a Community Living Center setting. The ideal candidate will be able to work effectively in a team-based setting, prioritizing resident needs and providing high-quality care.

This is a full-time position, working 12-hour shifts with rotation to other shifts as needed. The LPN will be required to work closely with other healthcare professionals, including Registered Nurses, physicians, and therapists, to ensure the highest quality of care for our residents.
