Job Summary

The Delaware Department of Health and Social Services (DHSS) is seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for providing case management expertise to the clinical monitoring process for the Diamond State Health Plan and Diamond State Health Plus Medicaid programs.

Key Responsibilities

  • Develop and implement comprehensive case management plans to ensure optimal health outcomes for patients.
  • Conduct regular monitoring activities, including joint/state visits and monthly assessments, to ensure contractual requirements are met.
  • Collaborate with healthcare providers, patients, and families to coordinate care and services.
  • Provide education and guidance to patients and families on health management and disease prevention.
  • Participate in quality assurance activities to ensure high-quality care and services.

Requirements

  • At least two years' experience as a Registered Nurse OR a Bachelor's degree or higher in Nursing and at least one year's experience as a Registered Nurse OR a Master's degree or higher in Nursing.
  • Possession of a Delaware Registered Nurse license OR multi-state compact license.
