We are seeking an experienced Inpatient Case Manager RN to join our team in the Hampton Roads region.
Key Responsibilities:
- Assess patients' social, psychological, cultural, environmental, and financial situations to identify needs and resources.
- Collaborate with patients, families, healthcare team members, and community organizations to develop effective transition plans.
- Use strong advocacy skills to identify needs and resources in the hospital and community.
Requirements:
- 18 months of recent inpatient RN Case Management experience.
- Ability to work assigned shifts in all units within the case management specialty.
- Flexibility to self-schedule at least 48 hours of prescheduled time per calendar month.
