Job Description:

The Care Navigator Healthcare Professional plays a critical role in facilitating patient care and ensuring seamless communication among healthcare providers. Key responsibilities include:

  • Conducting regular patient rounds to assess progress and communicate with patients and families
  • Coordinating with clinicians, administrators, and external resources to address patient needs and concerns
  • Assisting patients and families with resolving complex issues, such as financial, psycho-social, or administrative challenges

Required Skills and Qualifications:
