Job Summary:

VNS Health is seeking a skilled Nurse Care Manager to join our team. As a Home Health RN, you will play a vital role in delivering high-quality care to our patients in the comfort of their own homes.

Key Responsibilities:

  • Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' health needs and provide education on self-care management strategies.
  • Provide health-focused outreach to seniors who may benefit from supportive services.
  • Implement health promotion activities, such as health screenings, health assessments, and health care referrals.
  • Make telephone calls and home visits to assess patients' needs and support successful community living activities.
  • Make referrals to appropriate health services based on patients' assessed needs.
  • Provide health care advocacy by acting as a liaison between healthcare systems during times of transition.
  • Participate in quality assurance and education programs as requested by the Clinical Manager.
  • Refer patients needing direct patient care to their physician or to appropriate healthcare services.
  • Document all nursing interventions provided as required.

Requirements:

  • License and current registration to practice as a Registered Professional Nurse in New York State.
  • CPR/BLS Certification.
  • Associate's Degree in nursing required; Bachelor's Degree in nursing preferred.
  • Minimum of two years of experience working as an RN in LHCSA or CHHA.
  • Prior experience working with an aging population and providing clinical assessments of their health needs.
