We are seeking a skilled Licensed Practical Nurse to join our team as a Clinical Field Specialist. This role is responsible for delivering high-quality patient care within long-term care facilities, supporting behavioral health services provided by our practitioners.

Key Responsibilities
  • Analyze contracted facility residents' medical records to determine which residents can benefit from or need practitioner services.
  • Coordinate and meet with stakeholders at facilities to provide proposed medical plans to treat residents who are not being serviced and to expand or eliminate services to residents who are being provided services as needed.
  • Develop and implement plans to ensure the overall health of contracted facilities by providing services to all residents in need.
  • Analyze and ensure full coverage and optimal patient visits are achieved through the most efficient plan of treatment.
  • Review and ensure requirements needed for quality patient care are secured by analyzing the plan of treatment and engaging facilities to coordinate and update/revise the plan of treatment if needed.
  • Conduct on-site visits with facilities prior to scheduled provider visits to review charts and determine whether the patient treatment schedule should be adjusted based on the patient's medical needs.
  • Develop, suggest, and formulate operational policies to ensure proper services are provided to contracted facilities.
  • Provide advice to management on the most efficient way to communicate with contracted facilities regarding needs for provider services to residents.
  • Plan short-term and long-term business objectives at contracted facilities and communicate those objectives to management.
  • Analyze patient medication lists in online databases, EHRs, to ensure accuracy and identify any patient medication discrepancies and evaluate possible reasons and resolutions to presented issues.
  • Maintain patient EHR records to ensure completeness, including consents for treatment, facility patient face sheets, and other clinical care documentation as appropriate.
  • Assist with after-hours emergency situations requiring patching through to providers and assisting with hospitalization as needed.
  • Represent MindCare in handling and resolving complaints, arbitrating disputes, or resolving grievances of patients and/or facility concerns.
  • Provide coordination between providers and facilities to ensure patient healthcare needs are timely addressed.
  • Deliver clinical in-services to facilities and staff.
Requirements
  • Minimum 2 years of experience in skilled nursing settings.
  • Experience in behavioral health is highly desirable.
  • Active OH LPN license.
  • Strong oral and written communication skills.
  • Effective team player with the ability to collaborate across and outside the organization to support clinical operations.
  • Meticulous follow-through of delegated tasks, including follow-up with staff to ensure completion and quality.
  • Ability to excel in a fast-paced environment.
  • Strong organizational skills, including time management.
  • Computer skills, including Microsoft products and EMR/EHR.
