As a key member of our team, you will be responsible for delivering direct and indirect primary nursing care, working closely with our Home Care Coordinator RN, Elder Services Clinical Director, and Elder Services Medical Director.
Key Responsibilities- Provide Comprehensive Patient Care: Manage the health and well-being of assigned participants, including center and home care needs, and ensure timely and effective interventions to prevent hospitalizations and promote independence.
- Supervise and Mentor Staff: Supervise and guide both licensed and unlicensed personnel to ensure the delivery of high-quality patient care and services.
- Collaborate with Interdisciplinary Teams: Work closely with our interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement comprehensive care plans that meet the unique needs of our participants.
- Ensure Regulatory Compliance: Maintain accurate and up-to-date records, adhere to regulatory requirements, and ensure compliance with all relevant laws and regulations.
- Education and Experience: Graduate of a school of professional nursing program required; Bachelor of Science in Nursing (BSN) preferred.
- Licensure and Certification: Current Florida Registered Nurse (RN) license and valid CPR certification from the American Red Cross or American Heart Association required.
- Transportation and Communication: Must have a valid driver's license, proof of insurance, and reliable transportation to travel to participant homes and other locations as needed.
- Medical Clearance: Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
