Job Summary

Mercy Home Health is seeking a skilled and compassionate Registered Nurse (RN) to join our team in a PRN (as-needed) capacity. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working autonomously to assess, plan, implement, document, evaluate, and teach patients' needs and care.

Responsibilities
  • Assess patients' physical, emotional, and social needs, developing individualized care plans to address their unique requirements.
  • Implement and evaluate care plans, ensuring patients receive the highest level of care and support.
  • Document patient progress, communicating effectively with healthcare teams and families to ensure seamless care coordination.
  • Provide education and support to patients and their families, empowering them to manage their health and well-being.
  • Collaborate with interdisciplinary teams to ensure comprehensive care, addressing patients' physical, emotional, and social needs.
Requirements
  • Current RN license in the state of Missouri.
  • One year of experience in a related field, preferably in home health or hospice care.
  • Graduation from an accredited nursing school.
  • BLS CPR Certification through the American Heart Association.
