Evening shifts twice a week until 7:00 p.m.
OFFERING A SIGN-ON BONUS OF $5000 PAID IN TWO INSTALLMENTS - 50% AFTER 90 DAYS - 50% AFTER ONE YEAR
Key Responsibilities:
- Patient Care:
- Collaborate with team members to determine the need for treatment cessation and plan effective discharge, which may involve creating home treatment plans and arranging necessary equipment.
- Ensure a safe environment for all patients.
- Design tailored treatment programs for patients of varying ages based on physician referrals, evaluation results, and patient/family goals, while maintaining a productive schedule.
- Administer daily treatments, adjusting plans as needed to accommodate the evolving needs of patients.
- Complete all required competency assessments related to patient care.
- Engage in effective communication with team members, other professionals, and family members to enhance patient care.
- Select and instruct patients on the use of adaptive equipment.
- Oversee the patient care plan and guide assistive personnel in treatment implementation.
- Provide education to patients and families based on identified needs.
- Encourage participation in community education and marketing initiatives.
- Patient Assessment:
- Utilize appropriate assessment procedures to evaluate the abilities and limitations of patients.
- Conduct and interpret initial assessments to determine patient status across various functional areas.
- Set measurable short- and long-term goals.
- Monitor patient progress towards established goals.
- Complete all necessary assessment competencies.
- Documentation:
- Ensure all documentation complies with departmental policies.
- Daily Operations:
- Adhere to safety and infection control protocols in patient care and the workplace.
- Participate actively in departmental meetings and continuing education.
- Delegate tasks to assistive personnel while maintaining responsibility for patient treatment.
- May supervise physical therapy students after gaining sufficient clinical experience.
- Comply with patient satisfaction initiatives and uphold organizational policies.
- Perform additional duties as required, including scheduling and maintaining a clean work environment.
- Promote patient safety by reporting issues and engaging in safety initiatives.
- Participate in continuous improvement projects.
- Licensed Physical Therapist
- Bachelor's degree in Physical Therapy or higher from an accredited institution.
• Maintain continuing education as required by professional standards.
- Strong interpersonal and communication skills
• Proficient in various diagnostic and treatment techniques
• Understand the needs and behaviors of different patient age groups. - Eligible for licensure in the State of Illinois
• CPR certification.
- Potential exposure to hazardous materials and infectious diseases.
• Direct contact with bodily fluids.
• Capable of lifting, transferring, and assisting patients as needed.
• Availability to work evenings, weekends, and holidays as required. - Ability to lift up to 35 pounds without assistance; for heavier lifts, appropriate equipment and assistance should be utilized.
