As a Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering personalized, comprehensive care to patients in the comfort of their own homes. Our team of dedicated professionals is committed to helping patients manage chronic conditions, recover from illness or injury, and regain their independence.
Key Responsibilities:- Provide skilled nursing visits to patients in their homes, assessing their needs and developing individualized care plans.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
- Conduct thorough assessments, develop and implement care plans, and provide ongoing evaluation and revision as needed.
- Coordinate with home health aides, physical therapists, and other support staff to ensure comprehensive care.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care and outcomes.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related field
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
