Role Summary:

The Physical Therapist (PT) is responsible for evaluating and reassessing the rehabilitation requirements of patients. The PT develops, executes, and oversees therapeutic interventions that encompass therapeutic exercises, gait training, range of motion assessments, maintenance therapies, heat treatments, baths, and other activities typically associated with physical therapy.

Key Responsibilities:
  1. Perform evaluations and reassessments of the patient's functional capabilities using diagnostic and prognostic techniques.
  2. Design, implement, and manage a tailored therapeutic program aimed at restoring physical function, enhancing range of motion, strength, balance, coordination, endurance, or overall functional capacity.
  3. Collaborate with the Rehabilitation Director or designated professional to ensure the therapeutic program aligns with the overall care plan.
  4. Instruct and guide patients in techniques and exercises to improve their physical capabilities.
  5. Monitor, document, and communicate the patient's response to treatment and any alterations in their condition or treatment plan.
  6. Educate other staff and family members on physical therapy activities they can safely assist with.
  7. Engage in patient case discussions to coordinate care plans and promote positive outcomes.
  8. Participate in ongoing assessments and evaluations of the agency through quality assurance initiatives.
  9. Supervise Physical Therapy Assistants treating evaluated patients and be available for consultations.
  10. Participate in wound care as credentialed, following agency policies and procedures.
Physical Demands:

Ability to see and hear adequately to understand written and verbal communication. Must be capable of working a minimum of eight (8) hours per day while performing various tasks including driving, standing, sitting, assisting patients, writing, and speaking.

Requirements:
  1. Graduation from an accredited Physical Therapy program.
  2. Current licensure in the state of Texas.
  3. Minimum of two years of relevant experience, with preference for home health experience.
Travel Requirements:

Regular home visits and patient care, along with periodic office meetings. Reliable transportation and appropriate liability insurance are necessary.

Adaptability:

Must be flexible to adapt to client needs and changing schedules.
