Home Health Occupational Therapist

BAYADA Home Health Care is seeking an experienced Occupational Therapist to join our team in Edgewater, New Jersey. With a focus on providing high-quality care and support to adult and geriatric patients in their homes, this role offers the opportunity to make a meaningful difference in people's lives.
Job Description:Job Summary:
- Provide comprehensive occupational therapy services to patients in their homes, focusing on promoting independence and improving quality of life.
- Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes and enhance patients' abilities.
- Educate patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
Responsibilities:
- Conduct thorough evaluations of patients' functional status, identifying areas for improvement and developing tailored treatment plans.
- Implement occupational therapy interventions, such as exercises, equipment recommendations, and environmental modifications, to enhance patients' abilities and promote independence.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
- Maintain accurate records of patient interactions, progress, and outcomes, utilizing electronic medical records (EMRs) as needed.
Requirements:
- A current NJ Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
